and have provided answers to some of our most frequently asked questions

Office
Hours and Appointments
Our offices are open Monday through Friday, from 8:00 a.m. to 5:00
p.m. Appointments can be made by calling 269-979-6200.
Emergencies
If you have an emergency, please call 269-979-6200. Our phones are
answered 24 hours.
Insurance,
Billing, and Payments
Family Surgical works with many insurance and managed care plans.
With the large number of variations in insurance plans, it is the
patient's responsibility to verify his or her coverage and limitations
of the plan with his or her insurance provider. In some cases, insurance
coverage may not be applicable (e.g. cosmetic surgery or treatments).
We will file
your insurance claim for you and ask that all co-pay requirements
be made at the time of your office visit when care is provided.
Many insurance
providers require pre-authorization or second opinions prior to
surgery or hospitalization. We encourage you to take an active part
in this process by contacting your employer or insurance provider
prior to surgery.

Prescriptions
If you need a prescription refill, please call the office before
Noon to ensure availability the same day.
Referrals
We welcome the involvement of your family physician in your care
plan. After you have recovered from surgery, you will return to
your family physician for continuing medical care. If you require
additional services, we are always available to provide you with
our highly-skilled and personal touch.
Confidentiality
The doctor/patient relationship is a privileged one. Communication
between you and your doctor will not be disclosed to others without
